Medication at School
You must have your Dr. and parent complete the appropriate form
to have prescription or over-the-counter medicine given at
school. You can print the correct form below:
Medication must always be brought in a correctly labeled
prescription bottle, your pharmacy will provide you with a
second medication bottle for school if you ask for it.
